At least when they're active online you can gauge how many of them seems to be currently taking their medication. There was a reddit user like that : he'd make alts every week when he'd get permabanned. I lost track of his current accounts, but that fucker was in and out of mental institutions, tried to rape his mom, actually got a girl pregnant and openly talked that once his bastard daughter turned 12 she'd be "fair game"...
