If seriously, I have no idea and I don't know how accurate or up to date are those numbers; ex-USSR in general has unusually high levels of AIDS for sure, and e. g. Russian and Ukrainian prostitutes around here (I live in Eastern Europe myself, but not in ex-USSR) have always had that, hm, reputation of being AIDS-ridden for the past 20-30 years at least, but I have no clue about the situation in Estonia.

I know that Eastern Europe has that rep about HIV, but it just seems insane that Estonia, which has fared relatively well economically since the Soviet break-up, should be such an anomaly.

It could be that Estonia keeps its HIV-positive residents alive for longer than Russia does. iirc. the highest concentration is in the Tuva Republic or somewhere nearby and that most cases of HIV are out in the boonies, so medical care may not be as available to them. Estonia may also be better at recording cases of HIV whereas Russia's could be underreported.
