Hormonal changes in the womb and or genetic differences are the most likely cause for the difference in body vs brain in sex development. For example for trans men (female to male) they suspect two possibilities, one being an insufficient estrogen receptor the other being too little estrogen or too much testosterone being released during a certain time of brain development on the womb.
