I always felt like taxing the primary causes of a problem, in order to pay for programs related to relieving those problems would be an interesting way to do it. As far as healthcare goes I'd imagine this would lead to taxing food, especially relative to it's impact on health, and also taxing pollution. One would hope long-term this would create a negative feedback loop for these problems.
