The Internet (certainly with the help of Reddit) pushed one or two pictures of Trump tagged with idiot to the top of the search results. Then some news outlets made stories about it, populating the search results of the word idiot with even more Trump pictures. Almost all of these pictures you see are from articles only reacting to the fact that if you search for idiot Trump will show up as top result. These stories get more and more traffic and push even more Trump pictures to the top for the search results of idiot. It's nothing more than a circle jerk.
There’s a lot of factors here - an environment of constant praise for middling performance, inadequate early intervention for weaknesses in reading skills, incipient dementia, possibly congenital deficits. We can probably rule out things like chronic lead exposure due to socioeconomic status, and they don’t seem like cat people so it’s probably not toxoplasmosis.
I could see Melanie having a yorkie or some some other yappy dog, but I too can't imagine Trump would stand it taking attention away from him.

Essentially. Google sees a picture and finds keywords around it, links those key words to the picture and then weights it by traffic. Their algorithm is secret but this is roughly how it’s been done for a while.
It isn’t that some poor sod is sitting there finding pictures of cats and telling google that it’s a cat so when people search they see a cat.
