Hate to tell you - but you're still paying for it one way or the other - hospitals can't refuse that kind of service, they just pass on the costs to people with insurance by flagrant overcharging and drive up everyone's premiums.
Taxes would make the accounting a lot more efficient. A bullet in the head would, as well.
