It was still achieving the highest friendly fire rate by every possible metric once it had the same or better targetting systems than other aircraft in the same role. Its a bad plane that attracts bad people.

Cope. The A-10 only takes battle damage because it's a fat slow hog that attracts AA and manpads because it floats around at low altitude moving slowly and piggily while an F-16, F-15 or B-1B can just blast things with precision from 30,000ft and skirt out of danger at Mach 1.5 no problem

It gets worse when you remember what the A-10 was planned for (taking out Soviet tank columns). US military planners expected that in the event of a conventional war every last A-10 pilot and airframe would be dead within the first two weeks. !historychads
Like genuinely why would you respond to a military force whose entire air defense was focused around powerful SAMs with a no stealth no distance non maneuverable gunner plane as CAS.

It's unironically one of the worst fixed wing projects ever accomplished. It was developed at a time when the USAF was proving out laser guided systems, hi-tech interdictors like the F-111 were coming online, the the F-4 phantom was being fitted with f-14 targeting systems for strike roles. The A-10 was a terrible use of funds and was a huge distraction for the role of the air force, where they should have focused on fast-and-capable interdictors or cheap low-and-slow counterinsurgency prop places like the Super Tucano.
Instead we have a fat hog that costs as much as an F-15 and basically is a slow bullet magnet

It was intentional. The USS Liberty had sig int equipment that presumably was picking up on communications from nearby Israeli forces while they were fighting Egypt. Way above was also the NSA intercepting said communications and which documented a large part of the incident. Basically, the Israelis were killing hundreds of Egyptian POWs and noticed the US ship nearby. They flew out a recon plane that would've been easily able to identify that it was not Egytpian because they had nothing like it. Liberty radios out that they're an American ship. Then, the Israelis send out their aircraft which strafes the ship. Liberty sends more messages saying they're American. Then the Israelis send out two waves of torpedo ships to finish the job.
Body of Secrets by James Bamford is your best source on this. It only took decades of FOIA requests to get enough information to write about. Even then, he doesn't conclude that it was intentional, but he sure hints at it given all the warnings given off by Liberty, the incessant attacks by Israel, the lack of any Egyptian ship nearby nor which had the profile of USS Liberty, and the stakes at play of being discovered of killing POWs.
When the CIA had a recommended book list on their website, they listed three of James Bamford's.

Israeli's actions once they started to suspect that the ship was American don't make any sense in the conspiracy theory framework, that's autism.
Intentional lies include for for example people who "forget" to mention that USS Liberty strafed the torpedo boats first, after which they returned fire and torpedoed her, then stopped the attack once they got closer and saw the English designation on the ship.
