Except there's not a single utility that would use a vented manhole cover like that for an electric vault, nor would it go on that long without tripping somewhere upstream.

I dunno. The utility I work for uses plenty of vented covers, and we've had plenty of manhole fires that continue to burn, because a primary fault takes out the secondaries, and the fuses just see the fault as load, and never trip.
