I don't know the full story here, but I don't necessarily blame her. I doubt she left him cause he's fat, she knew that going in, if he's been on a constant cycle of food addiction and keeps relapsing then she might have just given up and wanted to cut it out of her life.
I mean there has been drama before about this, and it sounds like she has wanted to leave for a while. It sounds like he's been guilt tripping her with the fact that he can't live on his own. Now that he can it sounds like she felt comfortable leaving.
There needs to be some level of a disconnect between your life online and your IRL matters. Having meltdowns in front of potentially hundreds of thousands of people is not a great way to recover from horrific events.

He had to lose a certain amount of weight before he could get the surgery, and (given his food addiction problems) they probably needed a psychologist to affirm that he could follow the post-op requirements.
