Not quite. If you take a random 300 people based on real world location or something like that, you're going to fish out a couple of genuine schizophrenics on average.
If you make a post that reaches the frontpage of /r/pics and also appeals to schizophrenics especially, it will be seen by 10,000,000 people and 100,000 schizophrenics on average, which will then proceed to congregate in the comments and in further subreddits catering to them.
In the physical world a random community has overwhelmingly more sane people than actually honest to god mentally ill people. On the internet, because of self-selection effect, there are a lot of communities mostly populated by mentally ill people.
Our brains are not prepared to deal with that, and I pity the normies who find themselves in the Q-cult for example and get their brains reformatted because what the shit, if everyone around you firmly asserts an insane thing you'd better start believing it because all your ancestors who didn't were incurably insane.
I think his point is that some people are low-key crazy. Like you would never know it because they act normal 99% of the time or are just quiet but then you get them on a certain subject or put them in a certain situation and you realize they're wack.
I used to have a friend like that. I always thought she was a little awkward but she still seemed mostly normal. But after like six years I found out she was in love/obsessed with me to a really disturbing degree because she sent me a really long letter in the mail telling me as such (that was extremely vulgar and descriptive). I thought she was a pretty decent person before that.
