I don't think they ever mention it directly, but this is basically implied to the case. House's department is called "Diagnostic Medicine", which isn't really a department in real-world hospitals. This is explained as being something that was created just for House. So basically, because he's such a genius doctor, they made him his own department so that they could throw all of the difficult cases to him. It also seems pretty logical that his department would probably be a regional hub for difficult diagnoses. Local Hospital #6's best people are stumped, refer the patient to House, that sort of thing.

Also, I think the show takes place in New Jersey, so it's probably NYC metro area, so tons of people.

The show follows a pretty standard formula from the first season to the finale and pretty predictable.

However afaik the cases came from actual doctors all over the world and medical journals and they were hard to diagnose IRL to begin with. If you've watched the show you'd see that House only takes cases where standard diagnostics failed so I'm not sure what are you complaining about?

Also I'm not sure that would be bad PR. If a hospital had a genius doctor/team who solves cases no other doctor can solve, why would turnaround times be a problem?

And lastly and most importantly IT'S A FUCKING TV SHOW. Of course it's not completely realistic, I doubt they had any intention it to be realistic in the first place. That show is not about cases, it's about characters and their relations.

Because each patient is a unicorn case. They aren’t fixing broken arms and diagnosing people with pneumonia.

But this never happens in the real world.

Patients go to the doctor. The doctor diagnoses the MOST LIKELY illness and attempts to treat that. Maybe they send them to a specialist if they are not certain, and the specialist will do the same thing.

At no point is anyone ever diagnosed "hmm, that's a unicorn". They just find something that fits and assume it's that. If the tests come back negative or it doesn't respond to treatment then it's the next most likely candidate.

But this never happens in the real world.

Would you prefer to just watch a hospital?

Every other medical drama does this by just having a normal internal physician go "wait, something's not right" and figure it out.

It actually makes it more dramatic, because they can then have a conflict with some other doctor who's convinced their diagnosis was correct.

The "Sherlock Holmes" model works for crime, because cops sometimes don't have a good suspect. Doctors can find like 100 suspects just by going to WebMD with a the symptoms.

They deal with his unique attitude and style of solving the cases because he's saved so many lives over the years mostly for his own self amusement so he is respected by all despite all his flaws. The main pull of the show is that he's a necessary although borderline evil force of nature. You HAVE to deal with him or people die. This is actually a fairly known trope The Friend Nobody Likes. Other similar characters if I were to go to other media mediums characters like vegeta or guy gardner. Vegeta is a massive asshole but he's basically goku's equal barring a few episode gaps. You know what's really nice to have around? Two buddies like goku. Guy Gardner is also somewhat similar. If you don't know he's a lesser known green lantern. He hits on women, kills people sometimes, loses focus, easy to anger, one time he actually swiped a heroine's underage pussy and smelt his fingers only for one of the other heroines to legit threaten to murder him. Almost anything that takes place in House's clinic hours is something mundane, that's why you see him fighting having to spend time there. He's bored by standard medocine and wants medical puzzles. They show other doctors having to be normal doctors and House gets to ignore half of that because he's set up this special diagnostic group. They even show that there's a waiting list to be his next case when there isn't an emergency weird case.

The concept was to do a mystery procedural except set it in a medical setting. Problem is that a mystery is built around a huge event like a murder where it's plausible someone would get a week of work investigating on. Obviously this doesn't work in a medical setting, like what doctor have you ever gone to that spent more than 15 minutes filling out electronic paperwork on their laptop and yawning before dismissively prescribing you something their pharma reps told them was all the rage. Just imagine a doctor spending a week of concentration just on your case, ridiculous. Plus doctors get paid per service, that will House spent on one patient, like he gets 50 bucks for prescribing something at the end of it. He would go broke so fast.

Also FYI it's not like people investigating murders ever actually spend that much time thinking about the case either, they grab the nearest brown person and throw together a bunch of fake pseudoscientific forensic evidence to make it stick so they can close the case.

If you watched the show you'd know that he does have cases where he diagnoses the patient in 30 TV seconds.
