what actually happens is Aqua refuses breastfeeding completely.

the 12 year old girl was his cancer patient who showed him the Idol, he was just answering some dumb question of hers in a nice way. He took her death hard and coped by watching the same idol, Ai.

that same girl who died at least a year before he did is also reincarnated as his twin sister. she does not refuse breastfeeding.

@eva_getKONGed have watched the episodes so far and read a bit of the manga but @eva_getKONGed is not exactly a big fan. just thought this could use some clarification.
