The sole (Caucasian) operator of a "Hometown subreddit" encountered a situation where an African-American citizen of that hometown noticed that her comments were regularly downmarseyd. She spoke up.
His response was to take the issue to a moderator support forum - however:

He framed it as "Black woman making racist comments about white people".
The overwhelming response was an uncritical acceptance of this framing.
It took digging through offsite moderation archives to uncover the truth: she had made no racist comments.
She merely spoke up against being erased.

The upshot is that the black woman, erased by the quietly bigoted participants of the hometown subreddit and framed by the operator of the subreddit, concluded that Reddit was not a safe place for black people - and deleted her account.

The record high we saw was >70 false reports

Dogpiles of false reports, then automated false reports using throwaway sockpuppets switched out to avoid triggering an automated Abuse of The Report Feature heuristic, even going so far as to file false DMCA takedowns on posts that were nothing but a short descriptive title & URL.

Reddit's "expert system" AI triage that "surfaces" reports likely to be actionable also doesn't understand context or author-audience dynamics, which are crucial to knowing if a text segment is interpersonal / intergroup abuse, or factual discussion / reporting.
