Michael Jackson suffered from a similar body dismorphia as she does. When they look in the mirror, they see a giant nose looking back at them, not the two holes that we do. It's similar to someone with anorexia seeing a fat person in the mirror when they're actually a skeleton, or you seeing an attractive looking person in the mirror...
Take a look at some of the facial reconstructions they've done to burn and war victims, and you'll see what's possible. Both of these famous people could afford entirely new faces, but they choose not to.
