It's definitely not a thing, and you're right that there is zero widespread epidemic of 'evil dealers' putting fent in every drug they sell.
However, some fricking r-slur dealers DO make the stupid mistake of using the same scale to weigh out different sets of gear without properly cleaning up in between. It does happen.
Some peepeehead goes to juice up a batch of horse with fent, then some time later weighs out a bag of coke with the same scale. That's all it takes.
