If they can use tech to dodge the negative health effects of being fat, they might decide the appearance doesn't matter. Same reason Picard is bald instead of getting space minoxidil

I feel like star trek had alot of self discipline and conservation of resources, no matter how unlimited they were. Going bald to be frugal is one thing. Being fat bc there are no consequences is something totally different.
