The problem with restricting NHS treatment to people with certain health conditions /lifestyle choices is that the argument can apply to lots of other circumstances too.

Health insurance costs already vary based on age, even if you're healthy, and you can't do anything about getting old. Why shouldn't fatsos, smokers and r-slurs who like jumping from cliffs pay more?

Health insurance companies have actually cracked down hard on smoking. Smoking cessation shit is pushed super cheap and they find ways to basically penalize companies if it's confirmed they have smokers.
