Had an idea like this but it would be an actual simulated diaper that you'd have to change in realtime and it would put its current status as rslurred text at the end of all your new posts while under its effect. IE:
** DIAPER STATUS **
{user}'s diaper is nice and clean
Uh oh! {user} is a stinkycel! Xe needs a change right away!
It had the double whammy of being either really irritating (having to constantly visit the site to change like a neopet) or completely ineffective (you weren't online/didn't post while it was active so no funny mishaps) so I dropped it.
Maybe combine them so the diaper only gets worse if you try to shit award people?
