Used to work with prototype equipment that was probably worth $200k per unit. However, they were older prototypes that were being phased out, but we could still use them to devise testing methods on final production units. The oldheads there reassured me, "It's fine, it's a rite of passage if you break one."
Accidentally shipped out an undersized A/C unit for a job that called for a larger one. No one caught the frick up and it got installed. It ran full blast with crazy high power bills for like 2yrs before anyone figured it out. They (the contractor) swore off ever dealing with our company again and sued us for like 80k or so.
