Due to mostly a couple things:

-Bullets that land within 20m of the target are considered "accurate," the whole fricking plane shakes when the brrr do in fact be brrrrrring. There isn't much you can do about this when you "build a plane around a gun."

-Fricking crazy outdated tech suite and without a multi-BILLION dollar upgrade package per unit, most sortie pilots have to use actual binoculars to identify targets.

Aside from that, they're not very good for providing close air support. In Iraq you might see a video or two of an A10 come through and bail some marines out of a hot spot. For every one of those videos, there are a bunch not on camera where the danger close is too dangerously close.

Crayon munchers know this so when they hear an A10 is coming in to help, they'll cease suppressive fire and take heavy cover. The ragamuffins caught on to this and do the same when this happens. So the end result is that the BRRRRRR happens for a few minutes, the smoke clears, and then the battle recommences with maybe 10% casualties on both sides from the strike.
