54 y/o African American male had abnormal blood work and was referred to a hematologist who ordered a path smear. Abnormal follow up gets a referral to hematology oncology where labs get repeated and a path smear is sent for consultation at a different pathology lab. Looking through records, he declined to let us look at medical records since every doctor he saw was at separate hospitals that requires patient approval to release access to previous medical records so we just went in a circle of everybody knowing nothing and just ‘Hey, he's your patient now.'
What was the most likely cause for this. There are two answers, reason why this happened and the actual Dx.
Hint: he was given assistance on filling out paperwork correctly and the patient info gives away the most likely hematological issue.
