You can never be 100% sure with humans. Unless you think you are much better than the average guy at reading people, even a 0.1% chance would be pretty bad odds for me because raising another man's son is so catastrophic.

And people cheat a few orders of magnitude more than 0.1% of the time, even in cases where the partner is absolutely trusting of them.

You also have to understand that many women don't view lying about paternity as bad as men see it, so they have less scruples lying about it. And when they're at that point, they'll all rationalize how it's the right thing to do for the child.
