Real talk though the first two gynecologists probably didn't closely inspect her clit because they're supposed to entirely avoid that area due to concerns about sexual abuse. A gyno doesn't touch the clit or the clitoral hood at all during a normal gynecological exam, so it makes sense that if the smegma was under the hood and not visible it would be missed by most gynecologists. And I've never heard of this problem happening even amongst the hygienically disinclined so I suspect its pretty rare and not something they'd think to specifically look for.
