I think that that tilt is meant to symbolise / mimic Christ's head tilting as he was unable to hold it up / had died on the Cross.

I was lucky that I didn't have to fight to get diagnosed with IBS — my grandfather had been diagnosed with essentially the same syndrome.

A lot of trans women have to fight doctors & insurers to get IBS treatment. Because they view us as men, & the diffdiag for IBS forks at "s*x".
