I'm pretty certain Medicare literally already does this - you get some bonus for getting enough steps. I think it's pretty mild too, something like "get at least 5000 steps for at least 10 days per month" and you get some money added to a debit card if you do it.

I think it's on some kind of card they can only use at like pharmacies (think CVS or Walgreens, to cover OTC stuff from painkillers to bandages). Either way it does have a cash value and they definitely get it for some sort of steps goal which is comically low.

I actually wish they'd step up programs like this. Obviously there's gonna be plenty of fats who cheese it (like shaking their fitbit up and down to count extra "steps") but some portion will actually try to get healthier as a result and that's probably worth it in the long run.
