Honestly the pronoun stuff was clunkily implemented in the first place, like some sims have it, some dont but the game knows their pronouns based on s*x so this shitty feature is literally for the .1% of sims that have a peepee but want to be she/her.
And yes I play sims (not often) because my wife plays sims and (sometimes) we use it to get ideas for doing stuff around the house by designing it ingame first.
