Personal drama What the frick am I supposed to fix?

Going through notes I was asked to fix and I don't know what the frick is wrong. One of the highlighted parts is β€˜Patient is a pleasant female with prior family history of breast cancer (stage IV) in her natal sister and history of breast cancer (stage II) in adoptive sister.' I literally see nothing wrong. There were quite a few with typos that are easily fixed but is this seriously why you call back somebody that hasn't worked with you for over a year???

I'm fairly certain those are standard. I get the typos since it's preferred to be done by the physician writing and they're old enough to be an amendment and not a correction but that note I seriously can't think of anything wrong. Only thing I could think of is she doesn't know she has an adopted sister but she's in her 50s so should probably know that by now.
