Think like a doctor # just got into my main account

I'll be upfront in stating I have 0 expectations of anybody getting this correct.

Patient presents with right ulnar pain and leaves with a diagnosis of neuropathy and acute leukemia. What transpired to get to this point?

Some nurse ignored their issues while pretending to be a doctor on a gossip site, so the patient ended up with fricking cancer.

Look what you did.

Hmm... I went to a neurologist once, cause I had a slight numbness in some fingers. He made me do some hand/arm exercises and checked stuff like grip strength etc. Then I got some electrodes hooked up and they did some tests about nerve conduction velocity. Diagnosis was basically "could be a slightly pinched nerve or super early neuropathy". So I was recommended to take some vitamin b complex pills and do some stretching exercises. So something like that for neuropathy diagnosis in general.

No clue how you would end up with leukemia. Okay googled and I found a paper that said "Our case and literature review highlight, the fact that LGL leukemia should be considered in the differential diagnosis algorithm for peripheral neuropathy" and they did mention NCV as well. Yeah, but no clue. Probably a blood test, but dont ask me which. And they ruled out other neuropathy reasons (i know of alcohol & diabetes)

Holy shit somebody got in the proximity of an actual answer. Was writing up the actual answer and you are very close. LGL leukemia is incorrect in most cases. This case is more AMLM4 but general acute leukemia given presentation. We ruled out LGL AL because the tremors were from previous family Hx. NCV is of no concern in this case.

That is an interesting take and completely wrong. It's way easier to question things being missed.

You should probably do a CBC+D, document tremors and family hx of tremors and a few other physical evaluations I won't say as it gives the answer.

omg dude I'm on my phone. physical exam. Document tremors during this. neurological evaluation. Tinel's sign. Look for muscle wasting or weakness. CBC+D. Unexplained leukocytosis or blasts. More blood tests, imaging studies or bone marrow biopsy to check for leukemia.

I did half my job on a phone so that shouldn't be an issue outside of not being able to post notes. Actually pretty accurate but will say physical neuropathy is part of neurological evaluation. Flow showed a percentage of blasts in peripheral blood that did lead to a bone marrow Bx.
