The problem is the wait, if you have cancer or something serious they'll treat you immediately, but if you're looking for a doctor's appointment because of some stomachache or headache then you make a line and wait.

They never cover 100% but pretty much cover everything life-threatening or crippling.
Most doctors in the public systems don't charge a public insuranc, they work directly for the government and get paid a salary (they don't make anything close to what doctors in the US make), the public hospitals are owned by the government too (either on a provincial/state/local/federal level).
Then at least in Brazil there are also private hospitals and insurance for those who don't want to wait or expect something of higher quality, as well as doctors who attend privately but also inscribe on the SUS (the public health system) to charge extra fees and get some government gibs
That's what they do, and they don't even need to be doctors, there are people offering "ozone-therapy" and other bogus therapies getting money from the SUS
