It really was somewhat only a matter of time flying that many aircraft right next to each other, literally the night before there was almost a collision but they were just above the 1,000ft minimum so TCAS saved em.

From what I read there was some issue with traffic control telling her to keep an eye on one plane which she mistook for another plane as per the initial threads on reddit.
In that scenario depending on how many aircraft were covering the exact same space she was either r-slurred that day or it was actually a too much traffic flowing through the same space logistics problem.
Honestly I would go with the too much traffic at one spot idea on this one but the next one I am blaming a woman.

Already from what we know most of the blame goes to the Blackhawk which was 150 feet higher than it was supposed to be as well as being instructed to go behind the CRJ and she was the Pilot in Command of the aircraft.

No spring chicken as a helo pilot, but a hatchling when it comes to navigating a busy airport. I think a commercial fixed-wing pilot would need more than 1,000 hours to even be in the cockpit.
It seems like they were training for something complicated, and more dangerous than the real scenario for some reason. If we really needed to evacuate an official via Blackhawk, we could ground civilian flights.
