This is exactly how psychiatric abuse works. It doesn't require someone to 'plant' symptomsβit just requires someone with influence to frame a narrative. If a family member pushes hard enough, calls the right doctors, and ensures that all reports are written from a certain perspective, the diagnosis follows.
Psychiatrists don't diagnose in a vacuum. They rely on patient history, collateral information, and family reports. If that information is manipulated before I even walk into the room, the outcome is already decided.

Ironically everypony who watched this movie missed the point and the twist ending was a blind. The movie was about gaslamping and gaslamped the whole audience.
The story Leo's character supposedly concocted in his head (the island being run by ex-nazi's doing weird experiments) was actually true and he was actually there to investigate it. The whole charade they put on was not a therapy technique but a method of gaslamping to convince him that he was crazy (and drug him) and set up a catch-22 at the end where he either accepts he's crazy and stays on the island as a patient (therefore unable to report his findings) or voluntarily submit to a lobotomy. Either way preventing his interference into their activities.
The title of the movie is also a reference too gaslamping. "Shutter Island" refers too the lighthouse on the island which in those days was powered by gas
