What happened to good old fashioned r-slurred people?

I'm in my mid 40's. When I was younger I had a mentally r-slurred cousin. I would often see other r-slurred people in public too. What was up with them? Were they severely neurodivergent? Brain damage or traumatic brain injury? It wasn't Down syndrome because they have always been around too, like Corky. But I never see the classic r-slurred people anymore.
