My belief is that all "incest fans" actually don't get off to the fact that they're related. But rather the idea that they can only get a woman by having them as a captive audience.

I always figured it had to do with it being the safest and easiest to emulate of the taboo s*x fantasies as in take out the being related part and it's just consensual s*x between adults. Personally, I'm of the opinion that there are actually far fewer actual "incest fans" than "incest porn", because it's a fetish that they can check the box to with a sentence in the scene description and a couple lines during the shoot.
