The truscum belief at its core is that they have something wrong with their brain. This thing, whatever it is, makes them unable to accept living as the s*x they are. And although they can't escape that s*x since it's intrinsic to their body, they can try.
That's what all the hormones and surgery are for. To best ape the appearance and physiology of the other s*x. And they want everyone to play along and pretend that they really are the other s*x(in most circumstances) so that their brain problems don't cause them more pain. I think it can be likened to conditions like OCD where you need to live a certain way or your mind tortures you. And that's not their fault.

The thing about OCD though is that continuing the rituals and obsessive behaviors makes the disorder worse, the main treatment for OCD is actually forcing people to abstain from them as long as they can to wean them off of it.
