There's something called "scope creep" which is where nurse practitioners and physician assistants are getting more and more ability to do things previously only allowed by doctors including prescribing controlled substances (depends on state). This makes doctors BIG MAD since they require a lot more education and usually know what they're doing. All NPs/PAs are supposed to work under an attending physician.

Residents, who are full doctors but still in training, are the main ones taking umbrage to the scope creep as they have an attending as well.

The Burger medical system creates an unnatural shortage of Doctors by limiting seats in medical schools along with requiring an undergraduate degree in usually unrelated shit, meaning many Doctors are usually giga-autists or URMs.

They do this to protect physician salaries and other entrenched bureaucracy.

Burger med school is 33% longer than Swiss med school, are our doctors 33% better? Eh. Especially when a lot of care is provided by NPs and PAs anyway, since decreasing labor costs finds a way. This also pushes doctors to specialize since nobody want’s to take out 300k in student loan debt to be a GP in bumfrick indiana. (Which is why they’s so many foreign doctors doing lthe needful in Hometown, USA)

The US also puts a bunch of additional requirements on qualified foreign doctors from Canada and Mexico, because when NAFTA was negotiated they didn’t want an influx of Canadian and Mexican doctors in the US to increase supply and Canada and Mexico didn’t want to lose their doctors to the great satan.

Since NPs and PAs can prescribe and perform most of the functions that people go to the doctor for anyway, hospitals hire them (at 80k a year) instead of having another doctor on staff, because they gotta make the business work.

Nobody wants r-slurred doctors, but what has happened is that now people aren’t even treated by doctors.

Tl;dr a bunch of r-slurred regulatory shit and problems caused by lobbyists.

US politics is a framework of people screaming to cut other people’s r-slurred bureaucracy but to leave and maybe strengthen their own totally-necessary r-slurred bureaucracy to protect clients or whatever.

My family doctor is at a university clinic and thankfully they don't actually check you're enrolled cus I've been telling them I'm a student still for years lmao. All my friends are SOL. There's one urgent care afaik for the entire city. If you don't get to a walk-in an hour before it opens you will not be seen because the lines are so long. Sometimes the doctors just don't show up, can't tell you how many times I've seen posts about there being no doctors working at any walk-ins in town. 16 hour waiting times at the ER. The children's hospital also has 16 hour waiting times but thankfully they've diverted resources to a gender diversity clinic :marseyparty:

Canada has a population of 40 million, and a gov initiative currently is to get at least 500k immigrants a year coming in

So it's 1. Shit pay 2. Way too many patients to devote more than literally 10 minutes to them per appointment so they don't even get to feel like they're helping 3. Insanely overworked. And it's a cyclical thing because the more that can't handle it and move to the states, the worse the workload gets for the rest of them. I considered going back to school for my BSN but when I saw what that actually entails (writing papers and doing online β€œdiscussion”) I decided it’s just a scam for tuition money. My take as a nurse is I’m not a doctor, and have no desire to be one. When you work with doctors everyday you see it’s not as glamorous a profession as many think. 80% of my theory is anatomy, disease pathophysiology, pharmacology, with 20% social subjects e.g., Indigenous Health and Mental Health. We also have lab simulations weekly. We require roughly 840 clinical hours too, which means while everyone else is on their semester break, we are in the hospital. 3 years total, but very strenuous with a remarkable drop out rate.

Okay it sounds like you’re in a straight bachelors degree program which is different than what I’m talking about. Here in the states we have those but what’s more common is an associates degree (2 years) program with a state mandated exam at the end. During those two years we do probably a thousand hours of clinicals as well as weekly skills training and competence checks.

After this program you can return to school for what’s called an β€œRN-BSN” program to complete your bachelors degree, but hospitals do not require it and all that’s taught at that point is mostly fluff.

And by nursing theory I did not mean patho or anatomy etc, we had those as separate classes. Nursing theory meant like learning about Florence Nightengale and the models of nursing care etc., or nursing care plans. shit you learn for a test then never need again.

I’m a full RN working at a hospital with an associates. 90% of RNs in the states don’t have a bachelors as it’s not required by most employers and doesn’t really give you a pay bump.

The BSN is needed for some administrative jobs but they have no additional scope in a clinical setting compared to associate degree RNs.

Nurses should NEVER be trusted with anything real, 2 supplementary years and 3 months of β€œhands-on training” won’t change that. Nurses struggle to grasp the most basic concepts in medicine, if you ask them to actually describe the pharmacology and physiology behind anything, you will not get an answer. They cannot explain to you what medicine they’re giving actually does, they cannot explain to you what causes your disease and how this med or treatment will somehow help it, all they know is simple input:output of issue:remedy and have never had to deal with critical thinking.

Trusting an NP with your health is worse than trusting a black affirmative action student, DO NOT LET THEM TOUCH YOU THEY WILL ONLY MAKE IT WORSE.

Here in the states most of the specialized doctors are scum who will put as many fake diagnosis codes they think they can get away with on your chart. Some try to justify it as just trying to rip off insurance, but the patient gets hit with the bill too. They will also mislead patients into thinking they need extra procedures/specialized equipment to no benefit of the patient. The only real benefit is that the extra stuff isn't covered by insurance and when the patient agrees to use it, it means the doctor can charge whatever the frick he wants. Ex: Femtosecond laser for a patient who is getting cataract surgery with a standard lens. Without the laser the patient would have to pay nothing or next to nothing on most insurances, with the laser it's $1750 per eye. It doesn't make the surgery safer and it does not improve vision outcomes, we just heavily imply that it does (Its more precise and helps with lens placement). Technically the patient is getting minor astigmatism correction for the extra $1750 we charge, but we never tell the patient that is what they are paying for. We just tell them the doctor has recommended the laser and that the insurance doesn't cover the laser. If a patient pushes back hard enough we tell them that we will ask the doctor if it would be okay for them to not use the laser. All the real information IS present in our consents, but when our coordinators help patients understand the consents they heavily summarize what's in there and hope that the patient won't want to spend time reading all 7 pages. We have 10 different surgeons, some aren't scummy about this stuff at all and will properly inform the patients, the others are constantly trying to upsell.

Also I've come to think that most insurances are quite reasonable with payouts and what they cover. The only reason you get denied at in eye stuff is the doctors help staff being completely r-slurred. Meanwhile the doctors trying to rip off insurance are already making bank, but its still not enough in their eyes, they became doctors to get rich.
