3 car garage -- it's a lot easier and more convenient to have a 2+1 door arrangement than a single wide 3-bay door. I'm not sure why the third is set back, my mom's is the same way.

Weight of the door, mostly. It's only supported at the ends and the opener is in the middle -- a super-wide door will sag too much in the middle. They could reinforce it but extra reinforcement means extra weight.

You can't buy a three-bay door, at least not off the shelf for a residential home. The other option people do is three single bay doors, but the 2+1 arrangement is better. As for why it's set back, architects these days love to put random articulations into houses.

Right, they don't make them because it'd be too impractical.
When a torsion spring breaks, it's darn near impossible for the average person to lift a modern 1-bay door. 3 would practically need a forklift or a team
I'm happy with my 2Β½-car, but it'd also be nice to have indoor parking for the work truck
