Actually yes this is an r-slured policy. The anesthesiologist has no control over the length of the procedure. That mostly comes down to the surgeon and luck.
Youre lying on the table with your chest open and the surgeon makes a mistake or theres a complication that turns a 3 hour procedure into a 5 hour procedure. Is the anesthesiologist supposed to wake you up to make medicare happy? Maybe they should just leave the room and let you suffocate.
The only thing that will happen is costs will increase by the salary of the new accountants required to fill out this idiotic paperwork.

This mostly comes down to luck and PM competence.
Should I get paid extra if I open a PR at 6:30 instead of my usual 5 because I was assigned a ticket on the day before the sprint closes that took longer than my PM thought it would take? Probably not.
This is how salaried jobs work. My dad was a management consultant and sometimes he'd be home late too. He didn't demand overtime.
